Once we receive your completed form, Integra LifeSciences
will assign a Certification Number to
you and send you a training certificate for your files.
You will also receive your certification number
via email so please ensure you include your correct
email address on the form below. Your name
and information will be added to our Certified Physician
List. A certification number is required for
order placement.
